Description

Indications

• Acute treatment of gout attacks
• Long-term prevention of recurrent gout flares
• Management of Familial Mediterranean Fever (FMF)
• Off-label use in pericarditis or Behçet’s disease (under supervision)

Mechanism of Action

• Colchicine binds to tubulin, inhibiting microtubule polymerization in leukocytes. This prevents neutrophil activation and migration to inflamed joints, effectively reducing inflammation in acute gout and other inflammatory disorders.

Dosage & Administration

• FMF: 1–2 mg/day (adjusted per patient response)
  Note: Always follow physician’s prescription due to narrow therapeutic window

Precautions & Warnings

• Not for use in patients with severe renal or hepatic impairment
• Risk of toxicity increases with CYP3A4 or P-gp inhibitors
• Do not exceed recommended dose – toxicity can be life-threatening
• Avoid in pregnant or breastfeeding women unless clearly needed
• Monitor CBC, renal, and liver function during long-term use

• Nausea, vomiting, diarrhea
• Abdominal pain or cramping
• Bone marrow suppression (rare, dose-related)
• Neuromuscular toxicity with prolonged use
• Hypersensitivity reactions (rare)
